Yes. Our medical weight loss program is offered as part of Myriad Healthcare's Direct Primary Care model, which requires an active DPC membership. This is because safe, effective medical weight loss requires an ongoing clinical relationship — regular monitoring, lab work, medication adjustments, and direct provider access. The DPC model is what makes all of that possible at a flat monthly rate.

Medication costs vary depending on what is prescribed, the dosage, and your pharmacy. We do not mark up medications — you pay the actual cost of the medication. We will always discuss pricing with you before any prescription is written so there are no surprises.
Some medications may be available at reduced cost through pharmacy networks we work with. We are committed to helping you find the most affordable option that meets your clinical needs.
Candidacy for injectable weight loss therapy is determined on an individual basis based on your health history, labs, BMI, existing conditions, and medications. Not everyone is a candidate, and we take this evaluation seriously. We will never prescribe something that isn't clinically appropriate for you.

Yes — absolutely. Hormone evaluation is a standard part of our weight loss workup. Thyroid function, estrogen, progesterone, testosterone, cortisol, and insulin levels all play a significant role in weight management. If hormonal imbalances are contributing to your weight challenges, we address them as part of your overall treatment plan — not as a separate program.
Medical weight loss is a long-term endeavor, not a 30-day sprint. Most patients see meaningful results within 3–6 months of consistent treatment. The timeline depends on your starting point, your goals, how your body responds, and how closely you're able to follow the plan. We set realistic expectations from the start and celebrate every step of progress.
We hear this often — and it's a completely valid concern. Different medications work through very different mechanisms, and what didn't work before may not be what we'd recommend now. We always start with a thorough evaluation before prescribing anything, and we take your history with previous treatments seriously when building your plan. There is no one-size-fits-all answer in weight loss medicine.
Coverage for weight loss medications varies widely by insurance plan and is often limited or excluded. Because Myriad Healthcare is a DPC practice, we do not bill insurance for your clinical visits — those are covered by your membership. For medications, we can provide a prescription that you may be able to submit to your insurance, or we can help you find the most affordable cash-pay option. We'll always discuss this with you upfront.

You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this.
We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
Ask us to correct your medical record
You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this.
We may say “no” to your request, but we’ll tell you why in writing within 60 days.
Request confidential communications
You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.
We will say “yes” to all reasonable requests.
Ask us to limit what we use or share
You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
Get a list of those with whom we’ve shared information
You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
Choose someone to act for you
If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.

Treat you
